ALS License Plate

PDF version
*Design is pending NCDMV final approval. Available only for vehicles registered in the state of North Carolina.

Download application here!

Be the first on your street to have an ALS Research license plate! This is a real license plate issued by the North Carolina Division of Motor Vehicles (NCDMV) and 100% of proceeds will fund critical ALS research. The NCDMV must receive a minimum of 300 applications and fees before they will begin issuing this special plate.

Once we have collected 300 applications and fees, we will submit the fees and applications to the NCDMV for production. According to the NCDMV, once they begin production, license plates will be issued and mailed to individuals whose current plate is not expiring within the next three months and who do not have any stops on their vehicle record.  Those that do not receive a plate will receive a letter from the Division explaining what is needed in order for their special plate to be issued.

In addition to normal registration fees, there will be an additional annual fee of $20 for this special plate. For a more customized look, you may choose to personalize your plate with up to 4 letters, numbers or symbols for an annual fee of $50. $10 from each plate will support ALS research.

The ALS Research specialty plate will be added to the NCDMV website after we meet the minimum 300 applications required for production.
